Visiting professional medical services to Drymmau Hall
We often get asked by those people coming into our care, or by their families, just what sort of medical care can be expected by those who live with us at Drymmau Hall on a day-to-day basis.
We find there is often quite a lot of confusion because Wales has its own system for ensuring excellent health care in its care homes that is different from the English Enhanced Health Care in Care Homes (EHCH) model. Wales has its own system built around:
GP-led care
Every care home resident in Wales is registered with a GP Practice, and medical care is delivered through:
routine GP visits
urgent visits as needed
practice-based clinical reviews
All the people in our care are registered at the local GP practice in nearby Skewen which has excellent relationships with our residents.
Community nursing teams
At Drymmau Hall, as well as having a Clinical Lead, Cami, (even though it is not mandatory in Wales), and our Nurse Manager, Hayley (who is celebrating twenty years with us this year!) we are supported by district nurses who visit us to provide:
wound care
continence support
long-term condition management
palliative care coordination
Together, our in-house team and the wider community professionals provide the backbone of our clinical care at Drymmau Hall.
Multidisciplinary teams (MDTs)
All our residents have access to professional medical services as and when they need it and as and when needs change and they need access to different services. These teams are funded by the NHS and organised through local health boards. Our people can also have preferred private medical services brought to them, e.g. chiropody.
This model works well for our residents as the services come to them under one roof which negates the need to travel and access a host of different medical services at different places which is very hard for the old, the frail and those with dementia.
These NHS-funded services include:
opticians
dentists
physiotherapists
audiologists
dietitians
occupational therapists
speech and language therapists
All our people can benefit from these services, often assessed and treated on a one-to-one basis in the privacy of their own rooms.
Urgent community response
Wales has its own urgent community response system that is different from that in England, but guaranteeing similar rapid response services in the form of:
community crisis teams
frailty response teams
rapid nursing assessment
This ensures again that the people in our care at Drymmau Hall often do not have to undergo unnecessary hospital conveyance and can stay within the familiarity and security of their own home in a crisis.
